Liberator IIIA LV346Does anyone have the Form 1180 for the above? LV346 of 86 Sqn was ditched in 15 ft of water after the engines cut at 600ft following takeoff Thorney Island January 12, 1943. Hoping the Form 1180 indicates a root cause. Aircraft was to be salvaged by 49MU - did it fly again? For the record, Pilot P/O A W Dungate badly injured, copilot P/O R Harwood killed, Sgt J Murray missing, ? D I Dewar uninjured and Sgt A A Tulip uninjured. And... does anyone have the aircraft's squadron and individual codes? TIA. Robert ....Read More.robstitt on 25th May 2015 03:26:53
Liberator IIIA LV346Hi Robert, Air Britain Liberator Book has no history for LV346 after your incident so presume did not fly again. Damage after immersion in fifteen feet of salt water would probably not be worth the effort of repair. And Liberators were notoriously bad at ditching due to the bomb door configuration. As to codes, photos of the period show 86 Sqn Liberators with individual code letters only and no squadron code. The squadron was converting to type at the time of the incident. regards Peter ....Read More.PeterColwill on 26th May 2015 12:03:44

Board of InquiryNow have the Form 1180 for LV346 and it has a notation 'CoI' on the second page followed by the root cause of the accident. Now I think about it, I have seen the distinctive handwriting and CoI notation on other Form 1180s but didn't realize at the time what it was. Apologies for thread splitting re CoI/LV346!
