Can anyone help me in trying to establish what crew were on board Lancaster W4921 'UG - O' of No. 1654 Conversion Unit, when she crashed on September 17th 1943? The Pilot was Sergeant D (avid?) Matthews. As they took off the Lancaster swung, hitting a nearby 'battle' headquarters with the port undercarriage leg and outer engine, crashing back down and sliding to a halt. A fire broke out and the aircraft was consumed.

The accident card (and Chorley's volume 8 - HCU losses) makes no mention of anyone other than the pilot. Can anyone help to establish who else was on board, and their names, ranks and service numbers?
