    Default Battle Honours

I wondered if anyone could enlighten me (and hopefully others) on the subject of Battle Honours.

Any general information would be useful (or confirmation that the WIKI details are correct) but I would really like to know:

Did the Air Ministry decide the criteria for the award of a particular honour? If so, are these criteria documented anywhere?
Did the Air Ministry seek approval from the sovereign for the honour to be presented to each squadron?
Was there some form of document issued or presented to the squadron once approval had been gained? Do examples of these exist?

Any thoughts etc would be most welcome

    Regards

    Pete
