    Default WW 2 Circulation of Accident Report Forms - Diagram Date?

    Hello All

    I found this dealing with a history of WW 2 RAF Accident Reporting, but the Circulation of Accident Report Forms has no date?

    E.2 and E.3 Equipment Branches have been struck out.

    The only thing I can find, is that it likely dates before 1944, because of A.M.O. A.1347/1943 (A.589686/43/D.D.P.A.(1).-30.12.43.) refers to:-

    "1. A new Air Ministry directorate called the Directorate of Accident Prevention has been created in the Department of the Air Member for Training to co-ordinate all measures for the prevention of service flying accidents.
    2. The establishment of this directorate will not affect the independent functions exercised by the Chief Inspector of Accidents, who will retain the right to investigate any flying accident." ...

    "4. As from 1st January, 1944, all accident reports are to be sent to the Director of Accident Prevention and not, as hitherto, to the Chief Inspector of Accidents."
