    Default Report on Flying Accident or Forced Landing Attributable to Enemy Action (Form 765C?)

    Has anyone ever come across one of these from the Second World War period?
    I'd be very grateful if someone can point me to any examples.

    I'm not sure that the Form number is correct, by the way.

    Simon -- no, I do mean "attributable to enemy action'. That's one of the reasons I asked. The possible example I've seen says "non-attributable". Thanks for the transcribed examples.

    Unfortunately, the example jpegs in Ross's posts from October 2011 don't seem to be available any more.

    Dennis -- the reference I found was to a badly executed fake form, which came up for sale on eBay. I have demonstrated why I believe it to be a fake in an article on my blog
