    A report on the inquest appeared in the Gloucester Journal, May 9th 1931:

    ERROR OF JUDGEMENT CAUSES DEATH
    Inquest of R.A.F. Pilot
    A verdict of death by misadventure was returned at the inquest at Digby Camp, Lincoln, yesterday, on Flying Officer Stewart Sinclair Mackay, who was killed on Saturday.
    Flying Officer Southley said he saw Mackay making turns without enough bank. He stalled and crashed.
    Aircraftsman [sic] Josiah Swain, the other occupant of the machine, who was slightly injured, said this was his first flight as a passenger. After the crash he got out and tied to extricate Mackay. He shouted, "Are you all right, sir?" Mackay just groaned.
    Evidence was given of the aeroplane's perfect condition before the flight.
    Group Captain Sidney Smith said Mackay was a keen pilot, and his death would be a great loss to the service.
    The Coroner said the accident was due to an error of judgment by Mackay.

    AC Josiah Swain's Service Number is reported in earlier newspaper accounts of the crash, and is recorded as being 513149.

    As for the report of his demise in the crash in Hugh's post you linked to, I fear it was greatly exaggerated...
