The card that you have is an early style one.
The W:R:M is the damage category
From the RAF Museum list of abbreviations they were:
W = Write Off
R = Repair on site not possible
M(u) = Capable of being repaired on site by the unit
M(c) = Repairable but beyond the parent unit's capacity
There was also a Cat U which was undamaged.
The A:B:C was used as a very brief general cause identification. On the mid-1930s forms there was a box labelled Nature which had A, B or C.
Of the early war forms that I have all of the fatal accidents (all controlled flight into terrain) were A. The force landing / crash landing and the odd abandonment due to mechanical failure regardless of damage to aircraft seemed to be C
