Default AZARIAH Philips R.F.C killed 12.01.1918

Can any one help me with any information regarding Lt Azariah Philips who joined the R.F.C on the 5th March 1917 (so i am told)

he was killed on the 12.1.1918 apparently during a flying accident on Salisubury pLain whilst instructing another pilot.

Was appently at Thetford (no 12 T squadron) before he went to Salisbury, Old Sarum airfield previously known as Ford Farm airfield.

This is all the information I have and would be most interested in any information about this man or his service.

Any details held regarding the incident in which he lost his life would be very useful.
His service number if he had one
    The type of aircraft
The squadron he was part of and any aditional information about that squadron.

    Thank you very much
    Regards
    Brett

    Brett,

A copy of his service record is held at the National Archives at Kew, London - file reference WO 374/73571. There is also a possibility that some service information is held on AIR76 (RAF officer records but sometimes including scraps about those who died prior to the formation of the RAF on 1 Apr 18).
