Default Harvard FX381/FT344 and Oxford HN378/NM419 accidents

I am looking for details of crew members for two accidents at No. 16 (Polish) SFTS in 1945:
collision of Harvards FX381 and FT344 on 16 March 1945
    and
collision of Oxfords HN378 and NM419 on 14 June 1945.
Can someone with access to accident cards or other documents help?
According to various documents/publications one Polish pilot was killed in each of these crashes, which is unlikely.

Cummings, in The Price of Peace records that the pilot of Oxford HN378, W/O M Krwawicz was killed, the un-named pilot of NW719 abandoned his aircraft safely. 1 mile WSW of Kinoulton, Notts.

The unit ORB records the following for the June incident,

June 14th 1945, “Two Oxfords crashed near Kinoulton, Lt Erwawicz was killed and his body conveyed to Newton mortuary same day, 706454 AC Dabkowski bailed out and was admitted to S.S.Q.”

I don't have an entry for the earlier accident.

Thanks, I know the book. I'm trying to find out what documents said.

Thanks a lot! That's pretty conclusive.
Was AC Dabkowski Krwawicz's crew member or the other pilot?

Now, can someone provide info on the earlier incident? Was Krwawicz involved in the Harvard collision at all?

FX381 was in collision with FT344. His date of death is wrong on the CWGC , the GRO site says his death was registered in Bingham in the 2nd quarter of 45 which would tie in with an accident in June. 2 Poles were killed on the date noted at Newton though.
