I was looking for details a couple years ago. Afraid I didn't get much further than what's in that thread:

Does anyone have any details surrounding the death of Squadron Leader T. G. 'Tommy' Westlake of No.290 Squadron Royal Air Force, date of death given as 15 April 1944? He is listed in 20 July 1944 issue of Flight as "Died of wounds of injuries received on active service."
He previously served as a Pilot Officer on No.268 Squadron in early 1941 and I am after details of his death to include in the Squadron records.
Thanks,

Amrit,
Thanks for the lead to the previous thread. Certainly no definitive answer as to the reason for his loss. Might have to see if I can get someone to check the Squadron ORBs at Kew to see if there is any information in them.
