You will find a 20 page casuatly file for this airman already digitised on the National Archives Australia website. This link will hopefully take you straight to it http://naa12.naa.gov.au/scripts/imagine.asp?B=1067109&I=1&SE=1 If that fails simply search as a guest using the last name and service number only.

The file included a Report on Flying Accident form on page 10. That should fill you in with quite a bit of info.
