It’s a post war, circa 1950s, AM Form 1406
This was an extension to the F543/F1580 record card to list RAF Officer Record of Service for reserve service beyond the War Emergency.
Wartime service was transcribed from the F543 for both OR and Commissioned then the card was used to record post war service.

This form is not to be confused with RAF Form AM1180 which was Flying Accident Card. AM1188 was used prewar and into 1940 to record the pertinent details of forced landings.
