What caused the death of and/or what was the place of death registration for:

F/Lt Donald D. GRAY - 74109 - death registered Oxford - not found Flight archives.

Proposed aircraft losses for this day:

Oxford I - P1922 - 2 FTS - abandoned Brize Norton Village, Oxfordshire.
DH Rapide - AW115 - 8 MCU - crashed on take-off Cardiff.

This officer is on the missing page 215 from Flight archives, March 13th 1941

P McMillan sent me the equiv entry from The Times, he is listed as Died on Active Service.

The crash of Rapide AW115 apparently resulted in 3 injured airmen in a garden in Cardiff, none were reported as fatal.

Hello

F/Lt Donald D. GRAY - 74109 - the National Probate Calendar records that he died at the Radcliffe Infirmary, Oxford. The Graves Registration Report Form lists his unit as 6 M.U.

F/Lt Donald Dean Gray was my Great Uncle. In researching my family tree I ended up at this page via a Google search. I thought it might be of value to someone to have the following details from his death certificate following an inquest:

He died on 15 Feb 1941 as a result of injuries caused in a car accident when he ran into the back of a stationary lorry after lighting-up time. His wife (Alice Patricia Dean) died the following day. They left an orphan daughter. I believe he was a Pilot Officer at the end of WW1.

1293444 Leading Aircraftman Robert George Douglas DARBY

According to information gathered to remember the fallen of Bloxham, Oxfordshire, LAC Darby was killed in "a training accident at RAF St Mawgan" on February 15, 1941. I think the airfield, a satellite for RAF St Eval, was then known as RAF Trebelzue.

Donald Dean Gray 74109 ,attached to 6 MU , Was fatally Injured in a road accident at 19:40 Hours on the Witney to Oxford road near Whitney, being dead on arrival at the Radcliffe Infirmary of multiple injuries

Robert George Douglas Darby 1293444 , attached to Number 8 ITW , fell whilst climbing a cliff near the Atlantic Hotel at Newquay at 17:00 Hours. His body was recovered from the sea the following day at 14:20 Hours

William John Morgan 1264925 , attached to Number 12 SoTT , was killed in a MT accident when the lorry in which he was travelling overturned
