What caused the death of and/or what were the places of death registration for:

ACW2 Ivy M. COOK - 440371;
AC1 Albert V. HAWKINS - 914129;
AC2 William A. SLEVIN - 1261096;
LAC Robert F. WHITE - 1010370, and
AC2 Frank WILLIS - 1263989.

Proposed aircraft losses for this day:

Master I - N7758 - 5 FTS - crashed in forced landing Loxley Hall, Uttoxeter, Staffordshire.
Oxford II - P1093 - 15 FTS - crashed in forced landing Yarnton Offordshire.
Tiger Moth II - N9242 - TTU - crashed in forced landing Whistlemere Dam, 6 miles SW of Abbotsinch.
Tiger Moth II - N9452 - 19 EFTS - crashed on take-off at night Hawarden.

Ivy Cook,19,is registered at Cambridge,Hawkins,23, is registered at Bingham, Nottinghamshire, a William P Sleven,19, is registered at Scunthoupe, Lincolnshire, a Robert White,21, is registered at Sleaford, Lincolnshire, Willis is registered at Leeds,Yorkshire

AC2 W P Slevin - "Station Headquarters Lindsey; at 00.30hrs died due to a motor vehicle accident." (Source: AHB).

ACW 2 Ivy May COOK died in hospital in Cambridge 21-4-41 after a short illness. Buried in Lowestoft (Suffolk) Cemetery.
I had originally thought she was the "serviceman" killed in a raid on South Lowestoft this date, but I am still trying to identify that fatality. Any help appreciated !

ACW2 Ivy M. COOK - 440371; No KOAS or DOAS detail is listed against her in Flight magazine but details above.
AC1 Albert V. HAWKINS - 914129; DOAS Flight Mag Jun 19th 41
AC2 William P. SLEVIN - DOAS in May 22nd Mag; note middle initial
LAC Robert F. WHITE - 1010370 DOAS Flight Jun 12th 1941
AC2 Frank WILLIS - 1263989. DOAS Died on Active Service
