Just to add to what Armit has out for V9403, here are the two entries from the ORB.

August 19th 1941, “P/O Hoddinott + LAC Chadwick Wireless Operator in Lysander 9403 took off on wrong course when heading for night co-operation over Rhyl, no news received all day. S/Ldr Blake carried out search in Leopard Moth. Crash located near Saddleworth the Pennines on 20.8.41. Both pilot + wireless operator badly injured.”

August 24th 1941, “LAC Chadwick injured in Lysander crash on 19.8.41, died as a result of injuries, in Ashton under Lyme Infirmary.”

Hoddinott's full name and service number are 45900 Frederick William Hoddinott.

There wasn't an entry in the 8 FTS ORB for the forced landing accident.

Checking the CWGC-cemetery registers and my database found another airman sofar missing in my files. What was the place of death registration for AC2 Joseph H. SEALS - 1450576 and could there be a match to any of the aircraft losses mentioned in my #1 message of this thread.

Regards and thanks for your help.
Henk.
