What were the places of death registration for:

A - Killed on active service:
AC2 Oliver J. BLANCH - 866300, and
Cpl Richard V. JUPP - 588026.

B - Died on active service:
AC2 Eric S. BARLOW - 1659134 - age 27;
ACW2 Marie O.H. HEWITT - 2136462 - 939/940 (Balloon) Sqn (Sheffield-Rotherham, Yorkshire), and
LAC Edwin J. MARTIN - 1297653 - 2750 RAF Sqn.

C - Proposed aircraft losses:
Beaufighter VI - EL400 - 2 (C)OTU - crashed in forced landing Great Hatfield, 4 miles SW of Hornsea, Yorkshire.
Beaufighter X - JM291 - 301 FTU - crashed on take-off Lyneham.
Master II - AZ733 - 17 (P)AFU - collided with Master DL218 near Millfield, Fardon, Cheshire (already as a caualty on AZ733: P/O [Pilot] R.J. BURRILL - RCAF J18208 - DL218 could land safely).
Tiger Moth II - T8001 - 6 EFTS - crashed in forced landing Earls Barton.

Crew of EL400 recorded in the ORB as 1096374 Sgt Spence (Pilot) and 1144696 Sgt Webb (Navigator), both survived. The ORB recorded the aircraft on the 10th June as EL300.

The 1180 for AZ733 (a dual card recording both aircraft) records 2 killed for AZ733, and names Warrant Officer Burril R78790 (as killed) and Flight Sergeant W. Lynn 1346445 (survived), pilot of DL218. Richard Victor Jupp, age 24, is recorded in the Tarvin Rural District in Cheshire West which covers the area the crash occurred in.

Blanch - not Eng/Wales as far as I can see
Jupp - West Cheshire
Barlow (No S) - Stafford
Hewitt - Wortley, Yorkshire
Martin - there is an Edward J (age 22) listed in East Elloe, Lincolnshire

Hello

AC2 Oliver J. BLANCH - 866300 - he's recorded on the Civil Registration Death Index as 'BLANCHE,' registered Westminster June 1943, aged 45. His birth was registered January 1898, Newcastle-upon-Tyne, as BLANCH.

Marie Olive Hazel Hewitt 2136462 was admitted to Wharncliffe EMS hospital Sheffield, dying the following day of peritonitis

Oliver John Blanch 866300 , attached to 95 MU , taken dangerously ill on 9 June, died in the Westminster hospital following a gastrectomy for stomach cancer

Eric Stanley Barlow 1659134 , attached to 16 MU, was dangerously injured at Pottal Covert, Staffordshire when struck by a vehicle starting handle ,dying in Stafford General Infirmary
