What were the places of death registration for:

Sgt (Pilot) Leslie A. CREENE - 1337499 - not found in Flight Globel.

Sgt (Airgunner) Reginald F. HAYNES - 1205271 - 57 Sqn (Scampton, Lincolnshire - killed in action; may have been killed on an aircraft that safely returned to U.K.).

ACW1 Kathleen M. LEWIS - 2008266 - died on active service.

Proposed aircraft losses for Sgt Creene:
Hurricane I - V6639 - 56 OTU - crashed on take-off 1 mile E of Kinnell.
Master I - T8636 - 9 (P)AFU - crashed on overshoot Errol.
Master I - N8073 - 55 OTU - crashed on take-off Longtown.
Master III - W8941 - 5 (P)AFU - crashed on take-off Ternhill.
Typhoon IB - DN557 - 59 OTU - crashed on take-off Milfield.
Oxford I - MP412 - 3 (P)AFU - dived into ground Merry Hill Farm, Butcombe, Somerset (known that one airman was killed in this accident).

Again, there is a Sgt L A Greene listed KOAS in the Sep 9th Flight magazine, there is no such name on CWGC, I think this accounts for Sgt L A Creene. his death is registered in norton District, Yorkshire.

This Norton district must be a shortened version of a long district name, there is a Norton Radstock district in Somerset, perhaps this might place him on Oxford MP412? Batcombe and Norton Radcliffe are not far from each other, 28 mins.

Haynes Welton, Lincs, age 35

V6639 was being piloted by a F/Lt William Brown Shimmons, recorded incorrectly in the ORB as 756687, LG has 85687. He was very seriously injured but seems to have survived and was transferred to A&SD Branch in 1944.

Your guess about Haynes may be a good one as the Welton district area where he is registered includes Scampton.As I am sure you have seen, Haynes is in Chorley Vol 9 but there is no a/c loss in the 1943 Vol, that includes his name, from a raid to Milan 15/16 June

Sgt (Pilot) Leslie A. CREENE - 1337499 - the Graves Registration Report Form lists his unit as 3 (P) A.F.U. His death is registered at Norton, Somerset, which, as suspected, links him to Oxford MP412.

UPGRADE to Gordon Meudall Philpott R 166797, he died in the station hospital at Number 7 SFTS, of acute domurulononephritis

Norman Walker 1474514 , attached to the Belvedere disposal depot , died at Woodstock Railway station, Cape Province, of multiple injuries sustained when he slipped and fell between the platform and a moving train he was attempting to board

Kathleen Mary Lewis 2008266 a cook attached to RAF Stannington , was admitted to the Sanderson hospital on 27 May where she died after undergoing an abdominal operation

Frank Smith 1268490 , attached to Number 25 SOR , was injured in a lorry accident on 25 June and was admitted moribund to Number 3 General hospital where he died within 2 minutes of admission of a fractured of the base of the skull and cereberal lesions

Frank Smith 1268490 ,was killed at 18:45 Hours near Saida, when the lorry he was driving, RAF 61535 collided with Army lorry WD 4464320 in which the driver Gunner L.W. Cook 41239, also died
