Hello,
I'm researching the career of S/L Charles Duncan Bremner Green D.F.C., from Oakville Ontario, who transferred to the RFC from the 164th Bn. CEF. His record records a crash January 13, 1919, where he badly damaged his ankle. During the Second War he was OC 5 SFTS, and while in hospital for a recurrence of his injury, he fell out a window and died as a result.
I'm hoping someone can identify the aircraft and which unit.

Yes, that is always one of my go-to sources.
There is a Casualty Card for the 1919 accident at the link below. The cause of crash was the Pilot's foot becoming jammed between the rudder bar and fuselage strut.
